Passports
APPLYING FOR A US PASSPORT
This Form should be used under the following situations:
- If you are applying for your first US Passport
- Your previous US Passport was issued while you were under the age of 16.
- Your previous U.S. passport was lost, stolen, or damaged.
- Your previous U.S. passport was issued more than 15 years ago.
- Your name has changed since your U.S. passport was issued and you are unable to legally document your name change.
Please Note: You must appear in person at the Circuit Clerk's office to apply for a passport under these conditions. When you arrive at the Circuit Clerk's office you will be asked to have prepared the following information:
1. Completed Passport Application
2. Evidence of US Citizenship. Which can be one of the following:
- Previously issued, undamaged U.S. Passport
- Certified birth certificate issued by the city, county or state.**
- Consular Report of Birth Abroad or Certification of Birth .
- Naturalization Certificate
- Certificate of Citizenship
3. Present Identification. Which can be one of the following:
- Previously issued, undamaged U.S. passport
- Naturalization Certificate
- Valid Driver's License
- Current Government ID (city, state or federal)
- Current Military ID (military and dependents)
4. A photo copy of the Present Identification (previously listed). This must include both Front and Back of all documents.
5. A payment of $110.00 to the US Department of State, and a payment of $30.65 that goes to the Circuit Clerk.
6. A 2 inch x 2 inch photo.
**Beginning April 1, 2011, the U.S. Department of State will require the full names of the applicant’s parent(s) to be listed on all certified birth certificates to be considered as primary evidence of U.S. citizenship for all passport applicants, regardless of age. Certified birth certificates missing this information will not be acceptable as evidence of citizenship. This will not affect applications already in-process that have been submitted or accepted before the effective date.
In addition to this requirement, certified copies of birth certificates must also include the following information to be considered acceptable primary evidence of U.S. citizenship:
- Full name of the applicant
- Date of birth
- Place of birth
- Raised, embossed, impressed or multicolored seal of issuing authority
- Registrar’s signature
- The date the certificate was filed with the registrar’s office (must be within one year)
